Freezer is nearby front entrance. Please remember to close freezer door completely. 

Please provide all information on head tags or sample bag, attach tag to head, place in provided trash bag, and deposit in freezer. 

Follow instructions on IDFG signage at site. 

Samples with incomplete or illegible information may be deemed untestable. 

Staff will be available during normal business hours to collect samples.
