Present a completed Disabled Persons Motor Vehicle Hunting Permit [PDF, 290 KB] form signed by a physician at any IDFG Regional office. Physician certifies the applicant is disabled and has severe mobility impairment per Idaho Code. The words "permanently disabled" on reverse side of Idaho Drivers license is acceptable in lieu of physician certification form.
